Reference Requirements
It is the responsibility of the applicant to secure references who meet the requirements listed below. Should a reference not meet these requirements, the reference will be deemed unacceptable, and therefore the application will be incomplete and may not be considered.
Only three references (the first three received) will be accepted
My references have known me for at least 3 months
My references are professionals in a position to make reasonable statements concerning my character, personal qualities, professional attributes, and clinical abilities (if applicable)
One reference must be a supervisor or manager that has supervised me in a clinical, research and/or professional setting*
One reference must have knowledge of my performance in a healthcare environment/setting* (*Note: one reference can meet both the manager and healthcare setting requirements)
I have not provided healthcare services to my references
I have not received healthcare services from my references
My references are not a member of my family nor a personal friend
Provide your references with your UofT Applicant Number and email address. This is necessary to complete the reference form.
Ensure your references complete the form by the deadline date. Please be sure to let your references know that we will not accept any documents (including referee forms) after January 31st (11:59 pm EST).
Applicants will be able to verify the BScPA Program has received the 3 completed reference forms approximately two weeks after each form has been submitted, via the admissions portal at join.utoronto.ca. References will receive a confirmation of receipt from the program.
Note: Applicants are responsible for following up with their references to ensure the reference forms have been completed and submitted by the application deadline of January 31, 2025 (11:59PM EST).

Once you submit the completed form, you are not able to make any edits.
References will receive an email confirmation of receipt from the program.
Should an applicant progress to the Virtual Multiple Mini Interview (vMMI) stage, we will be performing random reference audits. Please ensure all reference contact information is up-to-date.
