Full name, title, and professional designation of the nominee
Name, title and professional designation of the nominator and relationship to nominee
Up to 1000 character statement describing:
A summary of the known contributions supporting the nomination
The impact of these contributions on students
Judged by the Awards committee
Criteria to include any of:
Collaborates with others
Provides service to advance the PA profession
Demonstrated leadership in local, regional or national activities
Advocates for patients and the community
Participates in scholarly activities related to the practice of medicine
Demonstrates professional, respectful and ethical behavior
Winner is notified by email. Official letter and award provided at Fall Convocation events
