Note: applicants must provide their referees with their UofT Applicant Number and email address.
Referee Requirements
It is the responsibility of the applicant to secure referees who meet the requirements listed below. Should a referee not meet these requirements, the reference will be deemed unacceptable, and therefore the application will be incomplete and may not be considered.
Only three references (the first three received) will be accepted
My referees have known me for at least 3 months
One referee must be a supervisor or manager
One referee must have knowledge of my performance in a healthcare environment/setting
My referees are professionals in a position to make reasonable statements concerning my character, personal qualities, professional attributes, and clinical abilities (if applicable)
My supervising or managerial referee(s) have supervised me in a clinical, research and/or professional setting
I have not provided healthcare services to my referees
I have not received healthcare services from my referees
My referees are not a member of my family nor a personal friend
Provide your referees with your UofT Applicant Number and email address. This is necessary to complete the reference form.
Ensure your referees complete the form by the deadline date. Please be sure to let your referees know that we will not accept any documents (including referee forms) after February 1st (11:59 pm EST).
Applicants will be able to verify the BScPA Program has received the 3 completed reference forms approximately two weeks after the last form has been submitted, via the admissions portal at join.utoronto.ca. Referees will not receive a confirmation of receipt from the program.
Note: Applicants are responsible for following up with their referees to ensure the reference forms have been received, completed, and submitted by the application deadline of February 1, 2024.

Ensure you complete the form by the deadline date of February 1, 2024 (11:59PM EST).
Once you submit the completed form; you are not able to make any edits.
Referees will receive an email confirmation of receipt from the program.
Should an applicant progress to the Virtual Multiple Mini Interview (vMMI) stage, we will be performing random reference audits. Please ensure all referee contact information is up-to-date.
